The form has been obsolete for a while now. Instead, login to the Wayfarer website, go to the below page, and tap on the speech bubble icon in the corner to open up the support chat. One of the things you can do here is request location edits. It's much quicker than the form ever was (you usually get a result within 48 hours), and so long as your browser doesn't automatically delete history or cookies on that site, the chat history will remain so that you can see when you've got a response.
