I just received a Upgrade BUT they added it to a nomination that was not approved. WHY did they not add it to one on my pokestop that was approved??

Upgrades don't get added to nominations that have already been accepted or rejected, nor do they have any effect once the nomination has reached a decision either way.
An upgrade just gets a nomination through the voting phase more quickly.
In your Wayfarer settings you can choose whether you want upgrades to be automatically applied or not. This setting is turned on by default.
If it is on, then any upgrades you earn will automatically be applied to your oldest nomination that hasn't yet been accepted or rejected, or if you've marked a nomination as "upgrade next", it will be automatically applied to that nomination.
If automatic upgrades are off, then any upgrades you earn will be banked and you can choose which nominations to manually apply them to using the "upgrade" button.
