How to nominate a location

I'm trying to figure out how to nominate a place to be a PokeStop or gym but I can't seem to find where I do that. I did the tutorial on what makes a place good to nominate. It would be nice if the tutorial covered how to do it and where to go since without knowing how to do it, knowing what makes it a good spot is pointless. Can someone tell me where I can find the nomination form or page please?

Goto the "Settings / Uploads" menu in the PG App. There you will find a button with which you can propose a new Pokestop aka Wayspot.
