Are there any current known issues with Portal submissions not actually appearing in Ingress after they are approved in Wayfarer?
I have 3 super obvious ones (a baseball diamond, a wood carving, and a sports centre sign) that are nowhere near anything existing and yet have not populated in game (and no key or AP)
Is there a way to flag these portals (waypoints) in forums for a moderator or?

You can tag @NianticGiffard or @NianticAtlas to take a look but as @AnsonNIA-PGO posted there is some ongoing work to Wayfarer which may delay things.
Also a post on the Ingress forums.
As an update; I have expanded on the bug now (have somewhere between 10 and 15 Waypoints)
Whats weird is the maps do not match at all in game (yes I am aware Ingress Portals are allowed to be closer together than Pokestops this is not same issue)
Instead I have some nominations that appeared as Pokestops / Gyms in Pokemon and did not appear at all in Ingress. AND in same area I have some Portals that appear in Ingress but did not appear at all in Pokemon. It almost seems as if the Waypoints / POI are not being shared between games.
And many of these portals are FAR apart, some are kilometres away from closest potential conflict point.

Can you guys give any feedback on this particular stretch of land? Its really weird how things are populating in one game but not the other (I used Google Plus codes since can not tag the actual waypoints since many do not appear in game)

It clearly is adding Waypoints by game
Anything I am submitting in Pokemon becomes a Pokestop (or Gym) and does NOT populate in the Ingress Intel Map

I have 4 more examples now so it is either a bug or a "feature". I tagged the mods as suggested but am awaiting some sort of official response.
I had one 5 days ago that was submitted in Ingress, but only appeared in Pogo. Still not Ingress as of 5 minutes ago. I should clarify that is according to the intel map; not sure if it is there if I was to check in person.
It is far away from all other existing in-game point of interests.
Surprisingly, it was approved after only 3 days with no upgrade.

Please check out Danbocat's post for more insight into the issue you're bringing up. Bullet point 3 to be exact.
So many posts about this, but I can confirm mine showed up in Ingress after I thumbs upped the Pokemon Go photos. I got the keys but did not have my "portals discovered" count go up. Could not verify the AP, but don't think I got that either. It did not sync at the regular time for Pokemon Go, about halfway between syncs.

Bug trapped and confirmed (I found the solution in an Ingress discord group and came back here to confirm)
IF you have a Waypoint approved and it ONLY shows up in one game (Pokemon only not Ingress or Ingress only not Pokemon)
Go to the photo and select LIKE
You WILL get the portal key added to inventory and it WILL show up on Intel map
HOWEVER it will not get counted in your stats (does not show as a discovered portal)
Curious if anyone has tried liking a portal they did not submit? Do you get someone elses key then?
I had certain portals invisible for over a month and after likeing them they gave key within update time frame
