Is there a reason the form https://docs.google.com/forms/d/e/1FAIpQLScMmz3lD30kz7vc3XkxaHyJFggxhfmsuPd2XGaCidOV-yUtzQ/viewform asks for email address twice?
Is there a reason we have a form at all, given that Ingress has a "Report Invalid Portal" functionality? Is it because other platforms (PoGo, etc...) don't have similar functionality? Or is the Ingress functionality being deprecated?
"otherwise shouldn’t have been added" is rather broad, especially in the rapidly shifting sands of acceptance criteria. can you give some examples?

Having never used the form myself, I'm wondering if it is a new version or not. Specifically, the "reason for reporting" of "Uninteresting location" is something out of the normal reasonhs for removal.
Anyone from Niantic care to comment?
"Uninteresting location" is likely just because all of the "never should have been eligible" ones, such as a few grocery store chains. I suspect they'll be more willing to remove that type of nomination from this seeded database as opposed to their general refusal to remove Wayfarer-reviewed nominations.

These new Wayspots are exclusive to Pokémon Go. They do not appear in Ingress.
The Report Invalid Pokéstop/Gym function does exist in Pokémon Go, but you need to be within the interaction range of the wayspot to use it. This form exists because quite a few of these new Pokéstops and Gyms are in locations which are not safely accessible (such as in the middle of bodies of water, or on airport runways), or are places that are inappropriate (think adult-themed).
If you're not a Pokémon Go player in one of the countries listed on the form, then this form will not be useful for you.
