The situation you described is unfortunately a common one when there's only one person submitting and reviewing in an area. It's something we have discussed with Niantic. I have no idea where it is on their priority list to address.

"Using inside access" is pure speculation. If there is a pattern of something weird going on, then I would recommend someone looking into it. But for one single POI? No, that doesn't need an investigation.
