I think a mural on the outside of a toilet block is fine and not sensitive.
People do not expect privacy standing outside a public toilet block - they would want privacy inside but would not expect it outside.

To clarify my previous comment, when I mentioned pointing a camera at artistic decorations, I was strictly referring to exterior artwork (such as murals) photographed from a clearly public, non-obstructive position, with absolutely no restroom users or private areas in view.
As I noted, restrooms are delicate places, and pointing a camera at them is subject to strict conditions: it must be done in a way that never interferes with the original purpose of using the restroom, and each case must be considered individually on its own merits.
I also completely agree that even with an innocent artistic purpose, photographing locations where it could reasonably appear suspicious or make people uncomfortable should be avoided.
This does lean towards them being seen as sensitive locations.
This comes down to there being 2 trains of thought…
Opinion and Consensus.
Example: Lets say that an authorised Mural is painted on the rear of the toilet block. My opinion is that this would be OK but if the consensus stated Reject I would not appeal it.
Probably in 90% of the cases a clear NO. However some of the really old ones in cities like Amsterdam & Paris I would approve, I’ve seen plenty of people taking pictures of them, so I would consider them POI’s
For what it’s worth, I found this mosaic in a nature reserve which is on the side of a toilet block. This mosaic has a QR code on it, so it seems as though at least in this case that it is expected that people will be taking out their phones and pointing it at the mosaic even if it’s on the side of a toilet block 
There is an app called something like “Where is public toilet?”.
Like Wayfarer, you can suggest additions and corrections (and removals).
