Does anyone know what it means when a PokéStop shows as unexplored even though it has already been explored?
It can mean that it has changed into a gym and you need to restart your app.
There may be other reasons but I have seen that reason before.
(As a note, however, technically this does not relate to Wayfarer. Wayfarer is about the system that accept Wayspots, whereas this is about Pokemon Go and how Pokemon Go uses the Wayspots.)
