I got a POI approved on June 13th nearly one month ago but it has not shown up in Pokemon GO as active yet? Where did it go?

The most common reason is that it is in an already occupied cell…

It is as @SlimboyFat71 says, your nomination is in the same level 17 S2 cell as “The Shilton Arms”, so it is unable to become a Pokéstop or Gym. It’s also within 22 metres of The Shilton Arms, so it won’t be able to become a Power Spot either.
A couple of things here,in my view, need sorting out.
Your nomination should have been duplicated with the older Parish Church of St Andrews one. It’s a picture of the notice board at the entrance, but is clearly about the church itself. Your nice picture of the actual church could then have become the cover photo. Personally I would have placed the pin on the church.
If there is anything architecturally interesting about the bell tower, I would nominate that at the pin placement. Or if there is anything interesting at the rear of the church that might be submitted.
The Shilton Arms is closed
That should be removed. It is a large plot behind it. If it reopens if there is a play area at the back then that could be submitted.
There is also a Baptist Curch if it is still there, 2 football pitches,
And I think you need to fix the location of the charity bench you submitted as it seems to be in the middle of someone’s drive.

The Shilton Arms is re-opening next month under new management so I will leave that alone. Great suggestion to perhaps nominate the belltower or the Shilton Baptist church … thankyou.
Also I will relocate the pin on the memorial bench
