There are several ways to accomplish this. There are two methods for reporting linked at the top here. I find the “Reporting Abuse” form easier to use, and Niantic has said that using this form does not mean someone will automatically get in trouble for abuse:
The second form is more geared towards property owners, but you can still use it. Just pick the closest option you can, then explain the issue. Do not reply if an automated response asks for your credentials as property owner:
Either of these options allows you to submit evidence. A human will get back to you through email.
Help chat will sometimes handle removal requests, but usually refers me back to the second form. A help chat request does not generate emails.
In game reporting is accessible through the pokestop. You can access the menu by tapping the title, then the three dots. They have fairly recently added the option to submit evidence this way. A reply will come via email.
If a removal report is rejected, it can be appealed in Wayspot Appeals with more evidence, but I usually do not have to do that if I use one of the forms and provide evidence then.
The more updated one will need to be submitted separately.
